Credit Card on File Policy

Effective 9/1/25, our practice requires all patients to keep a credit card on file for secure billing and scheduling purposes.

  • Cards are stored securely through a HIPAA-compliant system.
  • You will be notified before any charges are applied.
  • Cards may be used only for:
    • Copayments or outstanding balances after insurance
    • Fees for late cancellations or no-shows (see below)

No-Show & Late Cancellation Policy

To ensure appointment availability and respect for provider time:

1. Missed Appointments or Cancellations with < 24 Hours’ Notice

    • A $50 fee will be charged to your card on file.
    • Exceptions may be made for emergencies at provider discretion.

    2. Second Offense

    • A second fee of $50 will be applied.
    • Future appointments will require verbal confirmation by phone at least 24 hours in advance.

    3. Third Offense (within 6 months)

    • Possible dismissal from the practice due to repeat scheduling non-compliance.
